Older adults with hypertension and post-stroke conditions require sustained family assistance in line with the developmental tasks of families in later life. This community service program aimed to provide education, improve families’ skills in monitoring blood pressure, and strengthen the family’s role in assisting older adults with hypertension and post-stroke conditions. The program was implemented over four months (August–December 2025) in Wirogunan Village, Yogyakarta, and Ngestirahayu Village, Central Lampung, involving 30 families of older adults with hypertension and post-stroke conditions and 10 elderly health cadres. The implementation method consisted of socialization, training, technology application, home-visit assistance, and evaluation with sustainability planning. The results showed that family knowledge scores increased from 60% to 89% (p<0.05), blood-pressure measurement skills increased from 45% to 80%, adherence to blood-pressure monitoring increased from 36% to 83%, and family independence improved from the moderate category (50%) to the good category (85%). Older adults’ mean blood pressure decreased by 10–15 mmHg after three months of assistance. The program successfully established family support groups for elderly care in both regions as a form of program sustainability. Family assistance based on family developmental tasks was shown to be effective in improving families’ knowledge, skills, and independence in caring for older adults with hypertension and post-stroke conditions.
